Suicide Bereavement Support

I acknowledge that I have read, understand, and agree to follow the Parent Support Group Meeting Guidelines, including expectations related to confidentiality, respectful communication, and group safety.

3. Voluntary Participation and Assumption of Risk

I understand that participation in a suicide loss support group may involve discussing emotionally difficult topics and may bring up intense feelings, including grief, sadness, anger, or distress. I voluntarily choose to participate and assume all risks, including but not limited to emotional or psychological discomfort that may arise during or after participation.

4. Release and Waiver of Liability

To the fullest extent permitted by law, I hereby release, waive, discharge, and hold harmless The Ben Moody Foundation, its directors, officers, employees, volunteers, facilitators, and representatives from any and all claims or liabilities, including but not limited to claims related to emotional distress, psychological trauma, personal injury, or other damages, arising out of or related to my participation in the support group.

I understand that The Ben Moody Foundation is not responsible for the actions, statements, or advice shared by other participants.

5. Confidentiality Acknowledgment

I understand that confidentiality is a core value of this support group. While facilitators will emphasize confidentiality, I acknowledge that The Ben Moody Foundation cannot guarantee or enforce confidentiality among participants, and I agree not to hold the Foundation liable for any breach of confidentiality by other attendees.

6. Communication and Email Consent

If I voluntarily provide my email address or other contact information, I consent to being contacted by The Ben Moody Foundation for purposes including, but not limited to:

Information about future support groups or events
Resources related to suicide loss and healing
Foundation updates, outreach, and promotional communications

By signing this waiver, I acknowledge and agree to the following terms related to my participation in a Support Group for those bereaved by suicide. Facilitated by The Ben Moody Foundation

1. Nature of the Support Group

I understand that this support group is peer-led and facilitated by individuals with lived experience of suicide loss. I acknowledge that facilitators and volunteers of The Ben Moody Foundation are not licensed therapists, counselors, psychologists, psychiatrists, or medical professionals, and that this group does not provide clinical therapy, medical advice, diagnosis, or treatment.

Participation in this group is intended to offer mutual support, shared experiences, and community connection. It is not a substitute for professional mental health care. I understand that I am encouraged to seek support from licensed mental health professionals as needed.
